[1] Take a new file of 300 pixels, 500 pixels, of resolution 72 dpi in the RGB mode.

[2] Start by creating a rectangle using rectangular shape tool.

[3] Press Ctrl+T. Right Click and select Perspective. Adjust the nodes to get the shape as shown.

[15] Merge all layers, duplicate that layer Press Ctrl+T. Right Click and select Flip Vertical and take the move tool place it like me.

[16] Create a circle using elliptical shape tool.

[19] Double click the layer to open layer style window. Apply settings as shown.
